This is an APPOINTMENT-ONLY clinic located at 23100 Cherry Hill Rd. suite 9, Dearborn, MI, 48124.
Clinic is open from 10 am to 3 pm for non-medical services and from 5 pm to 9 pm for medical services.

Eligibility Criteria:
- Patient must be uninsured or under-insured
- Patient must be less than 64 years of age
- Patient must have No Primary Care Provider
- Medicare patients who are unable to afford their co-pays on prescription medications may qualify for services upon case review
Patient Policy:
- Patient is required to sign a Patient Contract as well as fill out a Health History and Consent form upon first visit.
- Patient or guardian must provide a photo ID upon check-in.
- Patient is triaged for medical urgency or severity of need.
- Patient is also assessed for eligibility to qualify for other medical care programs or resources available in the area.
- Ability to accommodate patient is dependent on clinic staffing.
- If staff is unable to accommodate medical need, patient will be provided alternate health care options and referred to other medical resource and programs.
