SPONSORSHIP CATEGORIES
CORPORATE – Donation of $50,000 +
CORPORATE sponsors will have their name listed on the WALL OF HONOR at the clinic website for a period of 5 full calendar years from the year of the donation. Corporate sponsors will also be granted 2 complimentary tables at the C-ASIST annual fundraiser dinner, will be presented with the “Rising Above & Beyond Crystal Recognition Award”, and have their name acknowledged and listed in honor of their generous donation on brochures and other clinic-related informational and promotional material.
PLATINUM – Donation of $25000+
PLATINUM sponsors will have their name listed on the WALL OF HONOR Platinum section at the clinic website for 4 full calendar years from the year of the donation. Platinum sponsors will also be granted 1 complimentary table at the C-ASIST annual fundraiser dinner, and will be presented with the “Platinum Recognition Star Award”, and have their name acknowledged and listed in honor of their generous donation on brochures and other clinic-related informational and promotional material.
GOLD – Donation of $15,000+
GOLD sponsors will have their name listed on the WALL OF HONOR Gold section at the clinic site for 3 full calendar years from the year of the donation. Gold sponsors will also be granted 1 complimentary table at the C-ASIST annual fundraiser dinner, will be presented with the “Gold Appreciation Plaque Award”, and have their name acknowledged and listed in honor of their donation during the fundraiser dinner.
SILVER – Donation of $10,000+
SILVER sponsors will have their name engraved under the WALL OF HONOR silver section at the clinic site for the life of the wall. Silver sponsors will also be acknowledged for their generous donations on the clinic website for a period of 2 full calendar years from the year of the donation. Silver sponsors will also be granted 1 complimentary table at one of the C-ASIST semi-annual fundraiser dinners, will be presented with the “Making A Difference Appreciation Award”, and have their name acknowledged and listed in honor of their donation during the fundraiser event.
BRONZE – Donation of $5000+
BRONZE sponsors will have their name engraved under the WALL OF HONOR Bronze section at the clinic site for the life of the wall. Bronze sponsors will also be acknowledged for their generous donations on the clinic website for a period of 1 full calendar year from the year of the donation. Silver sponsors will also be granted 1 complimentary table at one of the C-ASIST annual fundraiser dinner, will be presented with the “Appreciation Medal Award”, and have their name acknowledged and listed in honor of their donation during the fundraiser event.
